Please read the following carefully before submitting.

1. I acknowledge the potential utilisation of traditional healing plants within the context of the shamanic/spiritual activities I am about to engage in. I understand that depending on the specific ceremony, certain plants may be considered a legal grey area under South African law. I voluntarily choose to participate, fully accepting any associated risks, and assume complete personal responsibility for my decisions before, during and after the ceremony.
2. To the best of my knowledge, I am in sound physical and mental condition and am not aware of any physical, physiological or psychological issues that could impair my ability to participate in the ceremonial practices.
3. I commit to promptly informing the facilitator if I experience or become aware of any concerns regarding my physical or mental health.
4. Should I be unable to attend the ceremony for health or any other personal reasons, I acknowledge that I will forfeit 50% of the ceremony fee.
5. I am solely responsible for the care of my personal belongings and for arranging safe transportation to and from the ceremony venue.
6. I understand that while personal expression is welcome within the ceremonial space, I agree to abide by the established guidelines of the facilitator, which are designed to uphold the safety, integrity and harmony of the group experience.
7. I acknowledge that the facilitator reserves the right to deny or revoke my participation at any time if they believe my presence could pose a safety risk to myself or others, or for another significant reason. I agree to follow all instructions provided by the facilitator.
8. I accept full responsibility for any damage I may cause to the facilities used during the ceremony.
9. I agree to complete all payments at least 48 hours prior to the ceremony unless alternate arrangements have been made and confirmed with the facilitator.
10. I understand that the facilitator reserves the right to cancel or reschedule the ceremony. In such a case, I will be offered either a full refund or the option to participate in a future ceremony.
11. I acknowledge that participation is voluntary and understand that ceremonial and plant-medicine experiences may involve physical, emotional and psychological risks.
12. I confirm that I am at least 18 years of age. If I am under 21, I understand it is strongly recommended that I attend with the awareness and consent of a parent or legal guardian.
13. I confirm that I have read, understood and voluntarily accept the terms outlined above. By submitting this form, I confirm that the information supplied by me is accurate and complete to the best of my knowledge.
